Penn State Schuylkill's Fall athletic teams will be able to get sports physicals on Thursday, August 30, 2018 from 8:30am until 10:30am in the Nurse's Office located in Health and Wellness Building 210C.
If you are a prospective member of the golf, men's & women's cross country, men's & women's soccer, or volleyball teams at Penn State Schuylkill, this is your opportunity to get a free pre-participation physical exam.
Please have forms ready and personal information completed prior to arrival.
1.Please have First-Year, Juniors and Transfers fill out Pre-participation Exam form link below
2. 2nd-Year players and 4th-Year players need to complete Medical History for Returning Student Athletes.
All Prospective Student- Athletes will need this step complete to be eligible to practice.
